Accessibility and quality primary health care

Abstract

Primary health care (PHC) is the most widespread type of medical care in the Russian Federation, where the most episodes of medical and preventive care to the population begin and end. In the Russian Federation, there is currently a significant shortage of primary care workers: doctors of the district service are 1.5 times less than the required number, average medical personnel working with them - 1.8 times less than necessary, paramedics - 1.9 times less than necessary. Such a deficit against the background of the population's growing need for medical care leads to a high workload of medical workers and to a decrease in the quality of PHC provision.

The article analyzes the problems associated with the provision of PHC; considers the measures for the development of this type of assistance provided in the National project "Health"; proposes additional measures and calculates the necessary resources for their implementation. The key proposals to solve the problems of primary care are: the elimination of the shortage of doctors by increasing their wages (the ratio of 1.5 to the base salary and additional 1.3 to the base salary of personnel working in the rural areas), as well as providing additional social benefits; changing the organization of PHC by switching to the model of General practitioner for the adult population; mass training of medical workers; modernization of payment methods of medical organizations providing PHC.
