That are intended to ensure achievement of the priority national goal of the Russian Federation's development - an increase in the Russian citizens life expectancy at birth up to 78 years. By the RF Presidential Decree of 7 May 2018 "On national goals and strategic tasks for the development of the Russian Federation for the period until 2024" the directions for achieving these goals are also defined.

As for National Healthcare system the directions of priority actions of different federal projects were divided into two groups: clinical and organizational projects. The first represent comprehensive set of measures for the most important clinical services, the second should create required conditions for treatment delivery system management, without which it is impossible to achieve target values of clinical projects.

Base proposals for organizational federal projects are developed by specialists of the Higher School of Healthcare Organization and Management (VSHOUZ). Responsible for their organization was the VSHOUZ head, holder of post-doctoral degree in medicine G.E. Ulumbekova. She also became a moderator of expert groups on clinical projects on basis of All-Russia People's Front and Russian Academy of Sciences, carrying out organizational and methodological guidance of the relevant work progress. These proposals were submitted to the Ministry of Health of the Russian Federation one month before the agency submitted its alternative projects.

The concept of the federal project "Increasing the accessibility of nearby medical care"

In the decree of the Russian President from the May 7, 2018 No. 204 "On the national goals and strategic objectives for the development of the Russian Federation till the year 2024" are defined to ensure the achievement of 4 objectives and 12 targets in the healthcare system. The structure of the National project "Zdorovie" (Healthcare) includes 8 Federal projects: "Primary medical care system development", "Circulatory diseases control", "Oncology diseases control", "Children's healthcare development including the creation of modern infrastructure of medical care for children", "Providing medical organizations with the qualified medical personnel", "Methodological guidance of the national medical research centers", "Creating the uniform digital loop in healthcare based on the Uniform State Health Information System", "Development of the export of medical services" (more details are presented in this journal above).

This article presents the concept of the Federal project "Increasing the accessibility of nearby medical care". The authors analyze the main problems with the availability of primary health care in the country today and propose the main tasks for solving within the framework of this Federal project as a separate project or supplementing similar existing projects.

The role of non-economic motivation in motivation system of medical staff in public medical organizations

Аннотация

Non-economic incentives are a significant part of the motivation system in public medical institutions. Limited financing, constantly changing economic environment force them to look for the new ways to increase motivation apart form salary increase. The key advantage of non-economic incentives is that they exert intrinsic motivation. The aim of the study was to evaluate the degree of utilization of the non-monetary incentives in public medical organizations, based on the perceptions of employees the significance of non-economic stimulus. 407 physicians and nurses from public outpatient and inpatient clinics were questioned with a specially designed questionnaire. The results showed, that most managers of public clinics are already applying non-material incentives. For each respondent, the non-economic motivation importance index was calculated. More than half of the respondents (68.3%) had high level of the index (≥8.0). There is a statistically significant difference between managing and non-managing positions in evaluation the significance of factors. For managers, the most important is knowing that the their work tis valued by society. For ordinary physicians and nurses the most important are good relationship with colleagues and the opportunity to apply professional knowledge and skills at work.
