Healthcare in Russia: development scenarios

This article assesses the state of the healthcare system of Russian Federation in the year 2015. Also the effectiveness and fundamental macroeconomic problems of the Russian healthcare system are described. On the basis of the analysis the priorities for the year 2016 are set. Three scenarios of healthcare system development depending from the three options of the public funding are given, along with the corresponding prognosis of the demographic indicators in the Russian Federation.

4P-Medicine as a new model of healthcare in the Russian Federation

Аннотация

Of decisive importance for the sustainable development of society and economy is the health of the population. The challenges facing modern healthcare, dictate the necessity of creating a fundamentally new model of medical care – 4P-medicine. 4P-medicine integrates the concepts of personalization (individual approach to each patient), predictively (creation of a probabilistic forecast of health), prevention (preventing disease), participatively (motivated patient participation).The 4P-medicine is based on scientific-technological and methodological advances in genomics, transcriptomics, proteomics, metabolomics and bioinformatics. The article discusses the concept of 4P-medicine, as a new vector of development of the health system and major groups ideological, organizational and methodological problems in the development of 4P-medicine. Demonstrated the advantage of the transition of medical-diagnostic model of medicine on the model 4P-medicine, aimed primarily at the use of efficient technologies for early warning in the selection of individual diagnostic, prophylactic and therapeutic agents with active and motivated participation of the patient.

Standardization of requirements to structure of diagnosis – a key element in improving morbidity statistics and causes of death in population

Diagnosis structure should include the following headings: "main disease", "complications of the underlying disease" and "comorbidities". Section "main disease" includes only disease (or condition), which was the reason for diagnostic and treatment activities during the last episode of care, and might lead to death by itself or due to its complications. This disease (or condition) is reported and encoded in Part I of the medical death certificate as the primary cause of death. Direct cause of death (or fatal complication) and so-called intermediate conditions, which are selected from section "Complications of underlying disease", but not encoded, are reported in relevant paragraphs of Part I of the death certificate. In case of comorbidities other important diseases (or conditions), which were the reason for medical care, or predisposed to death in case of lethal outcome, defined as competing, intercurrent and/or underlying, are reported in the diagnosis in additional category after section "main disease" and described, respectively in Part II of the medical death certificate with appropriate codes according to ICD-10. Comorbidities (or coexisting conditions) are not reported in the medical certificate of death and are not coded as ones not having causal relationship with main disease, and not influencing lethal outcome in case of death.

Methodology of the Total Cost of Ownership (TCO) in managing core laboratories enables to do detailed assessment of costs (direct and hidden) and to choose new technological solution to meet the needs of laboratories. That allows to use evidence-based real life management tool for calculating and controlling the costs. Experience of the "Citilab" shows how to use this methodology and to contribute a comprehensive assessment of costs when comparing the two technological platforms and choosing one which leads to the improvement of lab profitability.

What should we know about the complex "Ready for Labour and Defence"?

Health promotion and physical education especially in youth were the major priorities of the Soviet government. May 11, 1931 the unique Civil Defense Training system “Ready for labor and defense” (“Gotov k trudu i oborone” – GTO, in Russian) was created and immediately became an important tool in the promotion of healthy life style among Soviet people. In 1991 the GTO system was discontinued. March 24, 2014 President Putin decreed to re-establish the GTO system, which should consist of 11 levels and embrace all citizens aged 6 and above. The goal of the new GTO system remains the same: help people to develop their own active and healthy lifestyle strategies, promote physical education and recreational sports. The First Sechenov Moscow State Medical University in cooperation with the National Health League and the National Alliance of Sport and Medicine “Healthy generation” has developed an educational course the Healthy Lifestyle Instructor. Online modules and practical sessions cover different aspects of healthy lifestyle, physical activity, selfcoaching etc. The unique feature of this project is that all information in the modules is preselected and critically appraised by medical professionals, and then presented in a popular form.
