Goals management proposals to achieve life expectancy at birth equal to 78 years till the year 2024 in Russia


The article describes the approaches to reach the goal set by the President of the Russian Federation V.V. Putin (Decree of the President of Russia N 204 of May 07, 2018 "On National Goals and Strategic Objectives of the Development of the Russian Federation for the period till 2024"), specifically - increasing the life expectancy at birth (LEB) up to 78 years till the year 2024. The article con­tains a quantitative assessment of the factors influencing the Russian popula­tion health and the description of the conditions necessary to achieve the goal "LEB equal to 78 years". The article states the healthcare policy priorities, the list of programs which include the indicators for annual monitoring until 2024. The management system for the development and implementation of the National Programs and Projects is presented.

It is necessary to implement 2 main interrelated strategic directions: health pro­tection at the population level (interdepartmental programs for increasing the adherence of the population to a healthy lifestyle) and a set of programs to im­prove the accessibility and quality of medical care. First direction is the zone of responsibility of the relevant ministries, regional and municipal authorities. Second - of the Ministry of Health, the Russian Academy of Sciences and of the regional healthcare authorities.

Viral hepatitis as public health problem in the Russian Federation (a design program for the control and elimination of viral hepatitis)


The significance of all forms of viral hepatitis (hepatitis A, B, C, D and E) for the Russian Federation is obvious, it is determined by the relatively high morbidity and mortality associated with these infections (especially chronic hepatitis C and B). The development of fundamentally new methods of diagnosis, treatment and prevention makes it possible to proceed with the global control of viral hepatitis. The WHO Assembly made decision on the global elimination of viral hepatitis as a public health problem by 2030.

The article presents the methodology for developing a program for monito­ring and subsequent elimination of viral hepatitis in our country. The program is based on the recommendations of the WHO («Global Health Sector Strategy on Viral Hepatitis 2016-2021. Towards Ending Viral Hepatitis») and the many years of experience of the coauthors team to combat these infections. At the first stage it is supposed to give an objective assessment of the prevalence of viral hepatitis among different age groups of general population in the terri­tory of at least three subjects of RF. The main principles of the formation of the Russian program for control and eradication of viral hepatitis are proposed and strategic and practical steps for its implementation are presented.

Analysis of medical organizations expenditures as part of the compulsory medical insurance system in the Sverdlovsk Region

The article describes the results of an analytical study of medical organizations in the Sverdlovsk region. The goal of implementing federal and regional projects to take into account the actual costs of medical organizations for the provision of medical care should be to take into account the resources consumed and for each specific case of patient treatment. On the basis of the data obtained, it is possible to develop a unified federal methodology for recording the actual costs of medical organizations.

Using behavioral economics to design physician incentives that deliver high-value care


Behavioral economics provides insights about the development of effective in­centives for physicians to deliver high-value care. It suggests that the structure and delivery of incentives can shape behavior, as can thoughtful design of the decision-making environment. This article discusses several principles of behav­ioral economics, including inertia, loss aversion, choice overload, and relative so­cial ranking. Whereas these principles have been applied to motivate personal health decisions, retirement planning, and savings behavior, they have been largely ignored in the design of physician incentive programs. Applying these principles to physician incentives can improve their effectiveness through better alignment with performance goals. Anecdotal examples of successful incentive programs that apply behavioral economics principles are provided, even as the authors recognize that its application to the design of physician incentives is largely untested, and many outstanding questions exist. Application and rigorous evaluation of infrastructure changes and incentives are needed to design pay­ment systems that incentivize high-quality, cost-conscious care.
